GENERAL St. Bernadette School has the responsibility to keep educational records for each student which will reflect the inter-relationships of the physical, emotional, social, and intellectual aspects of the student's development. State and Archdiocesan guidelines are followed which govern the collection, maintenance, administration, and dissemination of student records with utmost care and responsibility. ACCESS Alumni and/or parents/guardians of minor students may have full access to and the right to challenge the accuracy of data collected. If parents/guardians disagree, they may add a written statement of disagreement to the file. No one but school personnel, alumni, and parents/guardians of minors are permitted access to student data without a subpoena or written permission of the parents. CUMULATIVE RECORDS Cumulative records are maintained for each student in the school. Upon the student's entrance into the school, the school record is started and the student's history throughout the school system is recorded upon it. TYPE OF RECORDS Educational records are maintained for each student. Information on student records ranges from basic factual data to highly sensitive reports. Different kinds of records require different handling for security, access, as well as different time periods for maintaining. The following records are maintained as part of a cumulative file for each student: Progress Records -Permanent Records (Form E122). The permanent records include information regarding admission and transfer data, name, birth date, address, standardized achievement test data, annual attendance data, sacramental history, and extracurricular activities. They are kept permanently. -Student Evaluation Form (Report Cards). A report card is on file for each year the student was in the school and the final card is kept permanently. -Class Attendance (Form E127). Records are kept for two years. Physical Health Records The following records are kept for one year after the pupil leaves the school. -Emergency medical card (Form E126). -Log of medicine administered. -Immunization record. -Athletic permit card. -Record of student's ability to participate in educational program (Phy Ed, Exemptions, et cetera). -Routine screening tests such as hearing, vision, scoliosis, and any follow-up information. Behavior Records These records include data collected by a professional person or team and are confidential. No behavioral records are maintained for more than one year after the pupil ceases to be enrolled in the school unless the parent/guardian or adult student specifies in writing that the records may be kept. St. Bernadette School will withhold the transferal of official student records (transcript of credits) for unsatisfied financial obligations. Only those records will be be withheld for the semester(s) or scholastic period in which the unsatisfied financial obligation was incurred. Health records will never be withheld. | ||||
